Since the day he was born.
He was always a narcissistic psychopath and he’s been involved in organized crime pretty much throughout his entire career. But I’ve seen interviews with him from a few decades ago, where he was lucid and articulate. The dementia came much later.

That said, the whole world is suffering from Trump’s dementia. I don’t think he’s suffering at all, or if he’s even aware of it.
